• Nominees must be employed by a Technology Company.

Please provide a description of this individual’s achievements and then, based on the following questions, provide a detailed explanation of why you believe this individual should receive this award.

• How has this nominee demonstrated leadership in the technology industry?
• How does this nominee’s recent achievements support future growth/success of his/her company and the technology industry in general?
• How has this nominee demonstrated that he/she serves as a "model" for others to follow?
• How does this nominee foster collaboration?
• How does this nominee represent the "Spirit of Endeavor"?
